Registration Forms

Registration Instructions:

1. Download, print and complete the appropriate program registration form.
2. Download, print and complete the Medical Release Form.
3. Return all completed forms to the Denver Kickers Youth Registrar.

4. Include payment for the correct age.

Uniform Instructions

1. Indicate the size of the uniform on the registration form.
• If necessary, contact Theresa Reed at 303-940-7649 for available sizes.
2. Kickers Kids in stock uniforms will be reserved on a first-come-first-serve basis.
• Once the inventory is depleted, we will order additional uniforms at registration March 15.
3. Include uniform payment if registering early.
